Where can I get Form 6 for Voter Registration?

It can be downloaded from the website of the Election Commission of India. Form 6 is also available free of cost in offices of Electoral Registration Officers / Assistant Electoral Registration Officers and Booth Level Officers of the concerned polling station areas.

If the age of the applicant is more than 21 years, is it necessary to provide an age certificate?
